When a referral is received, the team will decide if the individual meets the referral criteria. The client/family will be contacted and asked to complete an intake form and to sign releases of information so that copies of prior testing and relevant care can be reviewed. A parent navigator provided by Stone Soup Group will be assigned to the client/family and will assist with the completion of these forms. The collection of these records may take several weeks/months depending upon how quickly agencies respond to our request.
When all of the requested records are at Assets, they will be reviewed to determine what further evaluation is needed. Appointments with the diagnostic team members at their private offices will follow. The client/family members will make the evaluation appointments and pay for them at the selected team member's private offices. (The team coordinator will be in touch with the team member about the referral before the client/family sets up an appointment.)
After the team members have completed their work, a team diagnosis decision meeting will be scheduled. The client/family will know when that meeting is planned but will not be present at the meeting. The family will meet with member(s) of the team shortly after the diagnosis has been determined and will be given the diagnosis and recommendations for appropriate treatment and supports. A brief diagnostic summary will be provided at this meeting and a comprehensive final diagnostic report will follow within one month..
Clients/family members can expect to hear from diagnostic team representatives after they have received and tried to follow through with recommendations and referrals. The team is interested in learning about how the diagnostic process worked for the client/family and about any challenges and triumphs they may be having after the diagnostic process is completed. They also hope to identify any gaps in services so that these can be documented and drive changes to help make agencies and "systems" better for individuals with FASD in the future.
